Foster Home Agreement

By fillout out this electronic form the hereby agree to provide a Temporary Home Environment (Foster Home) for DFWMGRR Golden Retriever(s) in accordance within the terms and conditions listed below:

Responsibilities of Foster Home:

1. I understand and agree that the dog will remain the property of Dallas Fort Worth Metro Golden Retriever Rescue (DFWMGRR). Under no circumstances will I transfer the dog to anyone other than a DFWMGRR Representative without prior written permission from DFWMGRR.
2. The dog will reside at the specific address listed below.
3. I have a fenced yard, and the dog will be kept confined in said yard or other enclosure at all times and will never be allowed to run free.
4. The dog will spend the majority of its’ time inside my home.
5. I agree and understand that the dog must be made available each time he/ she has an adoption appointment while in my care.
6. In the event that non-emergency veterinary care is required while the dog is in my possession, I will get approval from DFWMGRR prior to having such veterinary care performed and furthermore agree to have such care provided by a veterinarian approved by DFWMGRR.
7. In case of medical emergency, I will attempt to make contact with DFWMGRR at the earliest point possible with regard to the dog’s welfare. All medical decisions are to be made by DFWMGRR.

Responsibilities of Dallas Fort Worth Metro Golden Retriever Rescue:

1. DFWMGRR will be responsible for all veterinary care and medical expenses subject to #7 and #8 above.
2. If at any time you are unable or unwilling to foster the dog, DFWMGRR will take immediate possession of the dog.
3. DFWMGRR will provide you with a Foster Coordinator responsible for answering your questions. You may also contact a Board Member at any time.
